An application for registration fee credit must be accompanied by:

1. (1) the current metal license plate(s) and license receipt issued for the destroyed vehicle;
2. (2) the negotiable certificate of title covering the destroyed vehicle; and
3. (3) evidence that the vehicle has been destroyed to such an extent that it cannot thereafter be operated on the highways.
