1. (a) The original petition, including any amendment thereto, shall include the following:

   1. (1) the name of the person seeking to bring about or prevent action by the department;
   2. (2) the names of all other known parties in interest, if any;
   3. (3) a concise statement of the facts relied upon by the petitioner;
   4. (4) a statement of the type of relief or action desired by the petitioner;
   5. (5) any other matter required by statute; and
   6. (6) the signature of the submitting party or the party's authorized representative.
2. (b) In a case involving a contract claim the original petition shall also include a reference to the claim document submitted to the district office, and a copy of such claim document shall be attached to each copy of the petition.
