An individual is considered to have submitted an application when the individual or the individual's representative, as appropriate:

1. (1) has completed and signed--including entry of electronic PIN--the ILS-OIB application form;
2. (2) has provided the information necessary to initiate an assessment to determine eligibility and service delivery; and
3. (3) is available to complete the assessment process to determine eligibility.
