The statement of charges and notice of hearing issued by the Agency shall contain the following information:

1. (1) The name and last known address of the person against whom the order may be entered;
2. (2) A short and plain statement of the reasons the Agency believes the person is operating a career school or college without a certificate of approval; and
3. (3) The date, time, and location of the hearing.
