1. (a) A complete application for an original certificate of approval shall consist of the following:

   1. (1) a completed application form provided by the Agency;
   2. (2) complete and correct financial statements, as specified in this chapter, demonstrating the school is financially stable and capable of fulfilling its commitments for training;
   3. (3) the application fee as specified in this chapter; and
   4. (4) any other revisions or evidence necessary to bring the school's application for approval to a current and accurate status as requested by the Agency.
2. (b) Schools shall fully satisfy the Agency application requirements within 90 days of receipt of the original application, or else the application may be considered withdrawn.
3. (c) If a school fails to respond to a request for additional information within 21 days, the Agency may withdraw the application.

4. (d) To reapply, a school shall submit:

   1. (1) a complete application as required in subsection (a) of this section, including applicable fees; and
   2. (2) an affidavit stating that the school will not reopen until it has been issued a Certificate of Approval.
