There must be a permanency plan for all children receiving in home family-based safety services from PRS and for all children for whom PRS has responsibility for placement and care, which consists of:

1. (1) the permanency planning goal for the child;
2. (2) the specific steps to be taken to achieve the goal, with responsibilities and time frames established for taking those steps; and
3. (3) a discussion of the efforts made to achieve the goal.
