1. (a) Upon intake, a file on each inmate shall be established. The file shall include:

   1. (1) name of inmate with aliases;
   2. (2) description;
   3. (3) gender
   4. (4) marital status;
   5. (5) address;
   6. (6) date of birth;
   7. (7) offense charged;
   8. (8) date of commitment;
   9. (9) previous criminal record;
   10. (10) record of injuries;
   11. (11) inmate property inventory;
   12. (12) disabilities warranting special accessibility consideration;
   13. (13) name, address, and phone number of person to be contacted in event of emergency;
   14. (14) the name of the delivering officer, and the arresting agency;
   15. (15) documents that purport to legally authorize the inmate's commitment.
2. (b) Upon intake, a medical record shall be established and shall be kept separate.
