1. (a) Departments, programs and facilities shall fully and promptly provide requested data pertinent to alleged abuse, neglect, exploitation, death and serious incidents to the Commission.
2. (b) The data shall be submitted in the electronic format requested or supplied by the Commission.

3. (c) The data shall include:

   1. (1) Alleged victim(s) name;
   2. (2) Alleged victim(s) personal identification number (PID);
   3. (3) Name of subject(s) of investigation;
   4. (4) Date of birth and driver's license or state issued identification number of subject(s) of investigation;
   5. (5) Date of alleged incident;
   6. (6) Time of alleged incident;
   7. (7) Date the alleged incident was reported to the Commission;
   8. (8) Type of alleged incident (i.e., abuse, neglect or exploitation (ANE), death or serious incident (SI));
   9. (9) Type of injury, if applicable;
   10. (10) Restraint related, if so, what type (i.e., personal, mechanical or chemical);
   11. (11) Disposition of internal investigation (i.e., Founded, Unfounded, Inconclusive); and
   12. (12) County generated case identification number.
4. (d) The data shall be supplied at least annually or as required by Commission.
