Prior to the release of each resident from the facility, the authorized officer shall:

1. (1) verify the identity of the person receiving custody;

2. (2) do one of the following:

   1. (A) verify the release authorization documents; or
   2. (B) document the release authorization in writing if a judge or juvenile probation officer authorizes the release by telephone;
3. (3) obtain a release signed by the individual receiving the resident's personal property;

4. (4) provide information to a parent, legal guardian, or custodian regarding:

   1. (A) all medication prescribed while the resident was in the facility that the resident is currently taking, and the name and contact information of the prescribing physician;
   2. (B) any pending medical, mental health, or dental appointments; and
   3. (C) any present concerns regarding the resident; and
5. (5) obtain a receipt signed by the person receiving custody of the resident.
