The facility shall obtain and record the following information at the time the resident is admitted into the facility:

1. (1) date and time of admission;
2. (2) name;
3. (3) nicknames and aliases;
4. (4) social security number;
5. (5) last known address;
6. (6) adjudicated offense;
7. (7) name of attorney;
8. (8) name, title, and signature of delivering individual;
9. (9) sex;
10. (10) race;
11. (11) date of birth;
12. (12) citizenship;
13. (13) place of birth;
14. (14) name, relationship, address, and phone number of the resident's parents, legal guardians, or custodians; and
15. (15) primary language of the resident and the resident's parent, legal guardian, or custodian.
