Documentation of the grievance shall contain the following elements:

1. (1) name of the resident;
2. (2) housing unit or cell;
3. (3) date of the grievance;
4. (4) grievance tracking identification;
5. (5) nature or description of the grievance;
6. (6) date and time of receipt;
7. (7) name and title of the person receiving the grievance;
8. (8) response or resolution to the grievance;
9. (9) date and time of the response;
10. (10) name and title of the person responding to the grievance; and
11. (11) space for a written request to appeal the grievance response.
