1. (a) The facility shall refer a resident classified as high risk for suicidal behavior to a mental health provider or mental health agency within 24 hours from the time the resident is classified as such.

2. (b) The facility shall maintain written documentation that the referral was made. The documentation shall include:

   1. (1) the name and title of the person who notified the mental health provider;
   2. (2) the name and title of the mental health provider or name of the mental health agency notified;
   3. (3) the date and time of the notification;
   4. (4) the method of notification; and
   5. (5) a brief description of the response provided by the mental health provider or a responsive document from the mental health provider.
