1. (a) On completion of a training course by an applicant for a license, the qualified handgun instructor who trained the applicant shall submit a class completion report to the department within five business days indicating only whether the applicant passed or failed.
2. (b) If an accidental discharge occurs during training or proficiency examination, the qualified handgun instructor shall submit a report to the department within five business days.
3. (c) Reports must be submitted using the forms approved by the department.
