- (a) Affidavits to lift suspensions on accident cases and installment agreements having to do with no suits filed within two years or judgments pending shall be filed on Form SR-60.
- (b) The date "such security was required" is the date appearing at the top of the order issued to the party required to deposit security.
- (c) A request that the department waive or permit the cancellation of proof of financial responsibility must be filed on Form SR-87.
