1. (a) If an emission inspection certificate has been lost or stolen, then the vehicle may be presented at a department waiver facility for inspection and applicable fees must be paid. The replacement certificate is valid for the remaining period of validity of the original certificate.

2. (b) Proof must be provided regarding the nature of the loss, destruction, or theft by either:

   1. (1) An affidavit describing the circumstances of the loss or destruction.
   2. (2) An offense report from a law enforcement agency describing the nature of the theft.
3. (c) Alternatively, the vehicle may be reinspected, with the certificate issued being valid for 12 months from month of issuance.
