Expiration of Appointment during Suspension

- (a) After expiration of a period of suspension in which the certificate of appointment has expired, a vehicle inspection station owner desiring reinstatement may request reinstatement by notifying in writing the appropriate service commander.
(b) An owner seeking reinstatement shall:
- (1) submit a properly completed inspection station application, Form VI-2;
- (2) submit a properly completed signature card, Form VI-13; and
- (3) submit the statutory Certificate of Appointment fee of $30.
