- (a) A licensee is required to notify the department of any change of name or address within 30 days and apply for a duplicate license when such change occurs.
(b) An application for duplicate will be accepted in any of the following cases:
- (1) when a Texas drivers license, including occupational drivers licenses and instruction permits, has been lost, destroyed, marred, or mutilated;
- (2) when an out-of-state, no-photo licensee returns to Texas, before or after the expiration of 45 days;
- (3) when there has been a change in any other pertinent information.
