All original applicants for drivers license or identification certificate must present either a birth certificate or other acceptable evidence of name and birth date. Other acceptable evidence of birth date shall consist of any of the following and shall be accepted when presented in lieu of a birth certificate:

1. (1) infant baptismal certificates;
2. (2) adoption papers;
3. (3) legal change of name as recorded in a court decision showing date of birth;
4. (4) United States citizenship papers;
5. (5) public or parochial school records;
6. (6) insurance policies in force for five years or more showing date of birth;
7. (7) passports;
8. (8) for members of the military and their dependents, a letter from the base adjutant or personnel officer certifying date of birth from their records or a military identification card;
9. (9) any confirmation of birth date acceptable in a court of law;
10. (10) hospital birth certificates;
11. (11) birth announcement in baby books;
12. (12) in case of doubt, three apparently reliable documents which furnish the same information may be accepted;
13. (13) any other evidence which confirms to the satisfaction of the examining officer the proper date of birth;
14. (14) when applicant is obviously above age 18, a valid or expired drivers license certificate issued by any state or country.
