1. (a) Claims for eligible expenditures within the allocations set by the state board shall be submitted on forms provided by the state board.
2. (b) To be eligible for reimbursement, claims must be submitted no later than 60 days following the close of the fiscal year in which the eligible expenditures occurred. Exceptions to this deadline can only be made by the state board or the executive director with permission of the state board on a case-by-case basis.
