An application shall include the following.

1. (1) General information. The application shall state the applicant's name and address, the statutory authority creating the applicant and under which the applicant currently operates, and the names of the directors, and, if applicable, the managing officer for the applicant. The application shall also contain a map and description of the applicant's jurisdictional area and a description of the extent of applicable irrigated land, dryland, and rangeland acreages and current agricultural practices on these lands. The application shall provide an assessment of current agricultural water-use efficiency and an estimate of possible improvement.
2. (2) Use of grant. The applicant shall provide a plan for use of the equipment to be financed with the grant, and an estimate of the benefits that it expects from use of the grant.
3. (3) Conservation commitment. The applicant shall provide a description of the applicant's commitment to water conservation. A district-wide conservation plan and program of work may be considered as sufficient evidence.
4. (4) Participation commitment. The application shall provide a description of the applicant's ability to raise other revenue and evidence that the grant will be used as a supplement to other sources of financing available to the applicant for the purpose the grant would fulfill.

5. (5) Equipment utilization. The application shall include:

   1. (A) a copy of a supplemental memorandum of understanding with the Soil Conservation Service of the United States Department of Agriculture to provide personnel to use the equipment to be purchased with the grant; or
   2. (B) evidence of the applicant's capability to otherwise provide personnel to utilize the equipment to be purchased.
6. (6) Quality assurance. An application for equipment to be purchased under authorization of §367.21(3) of this title (relating to Purpose) for testing and evaluating water quality shall also include a quality assurance plan. The plan will describe how samples are to be collected, transported, stored, and analyzed in order to assure that analyses will be valid. The plan shall also include a description of training or certification for all personnel involved and for calibration of equipment.
7. (7) Water quality data accessibility. If the grant is for the purpose of §367.21(3) of this title (relating to Purpose), the application shall also include an agreement to provide, if requested by the board, the sampling results and analyses for inclusion in the state's water quality data bases. The application shall include an assurance by the applicant that such data shall be provided the applicant, in a form and upon a schedule to be established in the grant agreement, no less frequently than every six months from the date that the sampling is initiated.
8. (8) Amount of grant. The application shall specify the amount of grant funds requested and a description or listing of all equipment to be purchased.
