Certificate of Insurance

A certificate of insurance for closure, as specified in §37.241 of this title (relating to Insurance for Closure), must be worded as in the Certificate of Insurance for Closure, except that the instructions in parenthesis are to be replaced with the relevant information and the parenthesis deleted.
