1. (a) Every wastewater system operations company must submit an annual report to the executive director within one year and 30 days after issuance or renewal of the registration. The report shall include for each wastewater treatment facility or wastewater collection system:

   1. (1) name, location, and mailing address;
   2. (2) permittee's name and mailing addresses;
   3. (3) commission permit number, if applicable;
   4. (4) dates of operation during the reporting year;
   5. (5) names of all operators employed by the operations company, including their mailing addresses, license classes, license numbers, and the name of each wastewater treatment facility or wastewater collection system for which the operators work or have worked;
   6. (6) licensed chief operator for each facility; and
   7. (7) any additional information required by the executive director.
2. (b) The information in subsection (c) of this section must be submitted with any application for a new or renewal registration, along with the appropriate fee.
3. (c) If a company is bought or sold and a name change occurs, the company must apply for a new registration.
4. (d) Once a year, political subdivisions that contract to operate wastewater systems must report to the commission the information required by subsection (a) of this section.
