All written communications, including medical reports and medical bills, provided by the health care provider shall include the following information:

1. (1) the claimant's full name, date of birth, address, and social security number;
2. (2) the workers' compensation number assigned to the claim by the commission, if known;
3. (3) the date and nature of the injury or occupational disease;
4. (4) the employer's name and address;
5. (5) the insurance carrier's name;
6. (6) the health care provider's name, address, federal tax identification number, and professional license number.
