The commissioner shall collect, and the person affected shall pay to the commissioner, the fees set forth in paragraphs (1)-(3) of this section. No fee shall be collected from an administrator for filing TPA Form 7, Notification of Exemption Form.

1. (1) Filing fee for processing an original application for a certificate of authority--$500;
2. (2) On-site visit examination fee as specified in the Insurance Code, Article 21.07-6, §8--$250; and
3. (3) Filing fee for annual report--$100.
