An eligible policy that is subject to this subchapter shall be reinstated, without evidence of insurability, on payment of past due premiums and interest if it meets the requirements set forth in paragraphs (1)-(4) of this subsection:

1. (1) it has been in force continuously for at least five years immediately prior to the date of lapse;
2. (2) all premiums have been paid during such period, or within the grace period;
3. (3) there is a subsequent unintentional default in premium payments caused by the mental incapacity of the insured; and
4. (4) proof and request for reinstatement are submitted within one year from the date of lapse.
