Collection of Data Necessary to Provide Report

Each health benefit plan issuer to which this subchapter applies shall collect the data required by this subchapter for each mandate set forth in §21.3406 of this subchapter (relating to Mandates for Which Data Must Be Reported) and shall prepare and file a report as required by §21.3407 of this subchapter (relating to Reporting of Required Information).
