28 Tex. Admin. Code § 19.1308
Using the forms prescribed and furnished by the board, a person who desires to be licensed as a risk manager in Texas must submit an application for risk manager's license to Risk Manager's Licensing, Licensing Group, State Board of Insurance, 1110 San Jacinto Boulevard, Austin, Texas 78701-1998. The license fee is payable at the time of application. As part of the application, the applicant shall furnish to the board any information relating to the applicant's identity, personal history, experience, business record, or other items as the board may require. All documentation and records submitted or maintained for the purpose of licensure or renewal are subject to audit or review by the board.
