Limited service HMOs are prohibited from:

1. (1) Excluding services required for pre-existing conditions which would otherwise be covered under the plan;
2. (2) Establishing waiting periods for coverage of pre-existing conditions; and
3. (3) Imposing a lifetime coverage maximum for any covered service or benefit.
