Situations in Which Name Applications Will Cease

A name will cease to be reserved in the following situations:
- (1) when a proposed HMO fails to request extension before the end of a 365-day name application period;
- (2) when an application for a certificate of authority is denied; or
- (3) when a certificate of authority is revoked or cancelled.
