1. (a) At any time an individual is restored to competency, the treatment team must collaborate with appropriate entities to facilitate:

   1. (1) continuity of care, including specific non-clinical services and supports needed by the individual after discharge, such as:

      1. (A) housing assistance;
      2. (B) food assistance;
      3. (C) governmental benefits;
      4. (D) clothing resources; and
      5. (E) other supplemental supports; and

   2. (2) the individual's discharge from the JBCR program to the individual's discharge setting, including:

      1. (A) the county jail;
      2. (B) the LMHA;
      3. (C) the LBHA;
      4. (D) the LIDDA;
      5. (E) other community mental health provider; or
      6. (F) the care of a responsible person.

2. (b) If the individual is determined to be unlikely to restore to competency in the foreseeable future or is not restored after completing the JBCR program, the treatment team must collaborate with appropriate entities to facilitate:

   1. (1) continuity of care, including specific non-clinical services and supports needed by the individual after discharge, such as:

      1. (A) housing assistance;
      2. (B) food assistance;
      3. (C) governmental benefits;
      4. (D) clothing resources; and
      5. (E) other supplemental supports; and

   2. (2) the individual's discharge from the JBCR program to the individual's discharge setting, including:

      1. (A) a mental health facility;
      2. (B) a residential care facility;
      3. (C) the LMHA;
      4. (D) the LBHA;
      5. (E) the LIDDA;
      6. (F) other community mental health provider; or
      7. (G) the care of a responsible person.

3. (c) If an individual is not restored to competency by the 60th day and is being transferred to a facility or OCR program, the JBCR treatment team must collaborate with appropriate entities to facilitate:

   1. (1) continuity of care, including specific non-clinical services and supports needed by the individual after discharge, such as:

      1. (A) housing assistance;
      2. (B) food assistance;
      3. (C) governmental benefits;
      4. (D) clothing resources; and
      5. (E) other supplemental supports; and

   2. (2) the individual's discharge from the JBCR program to:

      1. (A) a mental health facility;
      2. (B) a residential care facility; or
      3. (C) an OCR program.
