25 Tex. Admin. Code § 97.178 – Reporting

(a) Cases.
1. (1) All suspected or diagnosed cases of tuberculosis (TB), tuberculin converters, and tuberculin reactors shall be reported to the local health authority or a Texas Department of Health regional office.
2. (2) This information shall be reported using the Texas Department of Health Report of Case and Patient Services (TB-400) Form as follows. This form is available from local health departments, Texas Department of Health regional offices, or the TB Elimination Division, Texas Department of Health, 1100 West 49th Street, Austin, Texas 78756.

(b) Tuberculin tests. Aggregate numbers of positive tests, total tests administered, and total tests read shall be reported monthly to the local health authority.
