The following information, at a minimum, shall be reported for any specimen derived from a human body that yields microscopic, cultural, serological or any other evidence of AIDS, chancroid, Chlamydia trachomatis infection, gonorrhea, HIV infection or syphilis, including a CD4+ T lymphocyte cell count below 200 cells/microliter or a CD4+ T lymphocyte percentage of less than l4%:
- (1) laboratories: name, address, city, county and zip code of residence, date of birth (month, day, year), sex, race/ethnicity, date test(s) performed, type(s) of test(s) performed, result of the test(s) or CD4+ T lymphocyte cell count, physician's name, physician's/clinic's address and telephone number. Positive tests and CD4+ T lymphocyte cell counts below 200 cells/microliter or less than l4% shall be reported and;
- (2) others as described in §97.132 of this title (relating to Who Shall Report Sexually Transmitted Diseases): name, address, city, county and zip code of residence, date of birth (month, day, year), sex, race/ethnicity, diagnosis, stage of diagnosis for syphilis only, date test(s) performed, type(s) of test(s) performed, and result(s) of test(s) or CD4+ T lymphocyte cell count below 200 cells/microliter or a CD4+ T lymphocyte percentage of less than l4%, treatment provided, physician's name, physician's/clinic's address and telephone number.
