1. (a) All reports of cases shall be submitted to the department within six months of initial diagnosis or admission at their facility for the diagnosis or treatment of cancer. The following reporting timelines may be followed in lieu of the six months standard: reports for 1996 and 1997 cases may be submitted up to 24 months after initial diagnosis or admission; reports for 1998 and 1999 cases may be submitted up to 18 months after initial diagnosis or admission; reports for year 2000 cases may be submitted up to 12 months after initial diagnosis or admission. Beginning with cases diagnosed in year 2001, all reports must be submitted within six months.
2. (b) Data shall be submitted at least quarterly; monthly submissions are recommended.
