   2. (2) 21 Code of Federal Regulations Chapter 1, §1020.30.
   3. (3) Health, Education, and Welfare application for approval of use of methadone in a treatment program, Office of Management and Budget Form Number 57-R0096.
   4. (4) Health, education, and welfare medical responsibility statement for use of methadone in a treatment program, Office of Management and Budget Form Number 57-R0095.
2. (b) Copies of the referenced regulations and OMB forms are indexed and filed in the Food and Drug Division, Texas Department of Health Resources, 1100 West 49th Street, Austin, Texas, 78756.
