1. (a) All applicants for a license must meet the general standards in Chapter 603, Subchapter F, of the Act, and submit:

   1. (1) the board required application form;
   2. (2) payment of the required fee of $180.00, and applicable fees and surcharges, as applicable;

   3. (3) required documentation including, but not limited to:

      1. (A) an educational transcript;
      2. (B) a certified transcript of examination scores;
      3. (C) a current national certification;
      4. (D) a birth certificate or other similar proof of age;
      5. (E) Professional or Work History Evaluation forms demonstrating or relating to the practice of perfusion for the preceding 5 years from the date of the application;
      6. (F) National Practitioner Data Bank/Health Integrity and Protection Data Bank report (NPDB-HIPDB);
      7. (G) FBI/DPS Fingerprint Report;
      8. (H) documentation of alternate name or name change, if applicable;
      9. (I) arrest records, if applicable;
      10. (J) malpractice records, if applicable.
      11. (K) treatment records for alcohol or substance use disorder or any physical or mental illness impacting the ability to practice, if applicable;
      12. (L) military orders or DD214, if applicable;
      13. (M) evidence of passage of Texas Jurisprudence examination with at least a score of 75; and
      14. (N) any other documentation deemed necessary by the board to process an application.

2. (b) Applications are valid for one year from the date of submission. The one-year period may be extended for the following reasons:

   1. (1) delay in processing an application;
   2. (2) referral to the Licensure Committee;
   3. (3) unanticipated military assignments, medical reasons, or catastrophic events; or
   4. (4) other extenuating circumstances.

3. (c) In accordance with Texas Occupations Code, Chapter 55, military service members, veterans, and spouses must:

   1. (1) meet the general requirements as set forth in subsection (a) of this section; and
   2. (2) submit a completed application on the board approved form and all additional documentation as required, with the exception of the application fee.
4. (d) The board may allow substitute documents where exhaustive efforts on the applicant's part to secure the required documents are presented.
