(a) The board shall charge the following non-refundable, non-transferable fees:
- (1) Processing licensure application--$200.
- (2) Temporary License--$50.
- (3) Annual License Renewal--$150.
- (4) Duplicate license--$45.
- (5) Reinstatement following cancellation for cause--$200.
- (b) All licensure fees or penalties must be submitted in the form of a money order or cashier's check payable on or through a United States bank. Fees and penalties are not refundable. If a single payment is made for more than one individual permit, it must be made for the same class of permit, and a detailed listing on a form prescribed by the board must be included with each payment.
