1. (a) The board shall charge the following non-refundable, nontransferable fees:

   1. (1) Processing licensure application--$300;
   2. (2) Temporary license--$50;
   3. (3) Annual registration fee--$200;
   4. (4) Duplicate license--$45.
2. (b) All licensure fees or penalties must be submitted in the form of a check, money order or cashier's check payable on or through a United States bank. Fees and penalties are not refundable.
