1. (a) If the renewal fee and completed application form are not received on or before the expiration date of the license, the following penalties will be imposed:

   1. (1) one to 90 days late--one and one-half times the required annual registration fee;
   2. (2) 91 days to one year late--two times the required annual registration fee;
   3. (3) over one year late--licensee may not renew the license.
2. (b) The board shall not waive fees or penalties.
