The following words and terms, when used in this chapter, shall have the following meanings, unless the contents clearly indicate otherwise.

1. (1) Contact lens prescription--a written prescription that contains the following information:

   1. (A) the patient's name;
   2. (B) the date the prescription was issued;
   3. (C) the contact lens manufacturer, if needed;
   4. (D) the expiration date of the prescription, which shall be one year or more unless the health of the patient requires an earlier expiration date;
   5. (E) the signature of the physician or a verification of the prescription as described by Tex. Occ. Code Section 353.1015;
   6. (F) the total number of lenses authorized and recommended replacement intervals if the prescription is for disposable contact lenses;
   7. (G) the brand name or model type of the lens prescribed;
   8. (H) the lens power;
   9. (I) the base curve measurements; and
   10. (J) the diameter.

2. (2) Disposable contact lenses--soft contact lenses that:

   1. (A) are dispensed in sealed packages;
   2. (B) are sterilized and sealed by the manufacturer; and
   3. (C) according to the wearing instructions suggest the lenses be replaced at an interval of less than three months.
