In addition to any other application, registration, or renewal fees, the board shall charge the following late fee penalties:

1. (1) Physicians:

   1. (A) Physician's registration permit expired for 31 - 90 days--$75.
   2. (B) Physician's registration permit expired for longer than 90 days but less than one year--$150.

2. (2) Physician Assistants:

   1. (A) Physician assistant's registration permit expired for 90 days or less--half the registration fee.
   2. (B) Physician assistant's registration permit expired for longer than 90 days but less than one year--full registration fee.

3. (3) Acupuncturists/Acudetox Specialists:

   1. (A) Acupuncturist's registration permit expired for 90 days or less--half the registration fee.
   2. (B) Acupuncturist's registration permit expired for longer than 90 days but less than one year--full registration fee.
   3. (C) Renewal of acudetox specialist certification expired for less than one year--$25.
4. (4) Non-Certified Radiologic Technicians. Renewal of non-certified radiologic technician's registration expired for 1 - 90 days--$25.
5. (5) Certification as a Non-Profit Health Organization fee for a late application for biennial recertification--$1,000.

6. (6) Surgical Assistants:

   1. (A) Surgical Assistant's registration permit expired for 90 days or less--half the registration fee.
   2. (B) Surgical Assistant - registration permit expired for longer than 90 days but less than one year--full registration fee.
7. (7) An individual who holds a license issued by the board is exempt from any penalty for failing to renew the license in a timely manner if the individual establishes, to the satisfaction of the board, that the individual failed to renew the license in a timely manner because the individual was serving as a military service member.
