The board shall charge the following fees to continue licenses and permits in effect:

1. (1) Physician Registration Permits:

   1. (A) Initial biennial permit--$856.
   2. (B) Subsequent biennial permit--$852.
   3. (C) Additional biennial registration fee for office-based anesthesia--$210.

2. (2) Physician Assistant Registration Permits:

   1. (A) Initial annual permit--$272.50.
   2. (B) Subsequent annual permit--$268.50.

3. (3) Acupuncturists/Acudetox Specialists Registration Permits:

   1. (A) Initial annual permit for acupuncturist--$337.50.
   2. (B) Subsequent annual permit for acupuncturist--$333.50.
   3. (C) Annual renewal for acudetox specialist certification--$87.50.
4. (4) Non-Certified Radiologic Technician permit annual renewal--$130.50.
5. (5) Non-Profit Health Organization biennial recertification--$1,125.

6. (6) Surgical Assistants registration permits:

   1. (A) Initial biennial permit--$561.
   2. (B) Subsequent biennial permit--$557.
7. (7) Certifying board evaluation renewal--$200.
