A dentist must submit a written report to the Board within 30 days after:

1. (1) the death of a dental patient which occurred as a consequence of the receipt of dental services from the reporting dentist; or
2. (2) injury to a dental patient requiring admission to a hospital, as a consequence of receiving dental services from the reporting dentist.
3. (3) The receipt of the report shall not be considered in the nature of a complaint unless the Enforcement Committee determines that the circumstances surrounding the patient death should be investigated by the Board.
