Notification and Change of Address

- (a) Licensees shall maintain a current physical home and business address with the Board. A different mailing address may be provided in addition to the home and business address. Licensees shall notify the Board, in writing, of any change in address within 30 days of the change.
- (b) The notification shall be signed by the licensee and must include the license number.
