- (a) All applications for certification by examination shall be made on forms prescribed by the board and shall also be in compliance with board rules and with all applicable laws. Each applicant must also submit official transcripts showing compliance with the applicable education requirements.
- (b) Each applicant shall submit with the initial application, and as instructed thereafter, two references from certified public accountants with a current license and one personal reference who can attest to the applicant's moral character. The applicant may use his/her current employer in lieu of the personal reference.
- (c) Each applicant shall submit his social security number on the forms prescribed by the board. Such information shall be considered confidential and can only be disclosed under the provisions of §511.105 of this title (relating to Confidentiality) and the Act, §25.
