A fully written contact lens prescription as referred to in the Texas Optometry Act, §§351.005, 351.356, 351.357 and 351.607, concerning the dispensing of contact lenses by ophthalmic dispensers must include the following:

1. (1) Rigid contacts:

   1. (A) base curve, unless set by manufacture;
   2. (B) power;
   3. (C) diameter, unless set by manufacturer;
   4. (D) optical zone, unless set by manufacturer;
   5. (E) peripheral curve, unless set by manufacturer;
   6. (F) peripheral curve width, unless set by manufacturer;
   7. (G) secondary curve, unless set by manufacturer;
   8. (H) secondary curve width, unless set by manufacturer;
   9. (I) blend, unless set by manufacturer;
   10. (J) thickness, unless set by manufacturer;
   11. (K) color, unless set by manufacturer;
   12. (L) brand name;
   13. (M) material, unless set by manufacturer;
   14. (N) prescription date;
   15. (O) expiration date;
   16. (P) optometrist's signature or authorized signature;
   17. (Q) number of lenses;
   18. (R) substitution language.

2. (2) Soft contacts:

   1. (A) base curve, unless set by manufacturer;
   2. (B) power;
   3. (C) diameter, unless set by manufacturer;
   4. (D) color, if needed;
   5. (E) brand name;
   6. (F) prescription date;
   7. (G) expiration date;
   8. (H) optometrist's signature or authorized signature;
   9. (I) number of lenses;
   10. (J) substitution language;
   11. (K) recommended lens replacement interval.
3. (3) The authorized signature must comply with the requirements of §279.14(b).
