1. (a) A registered nurse whose original certificate of registration or reregistration is lost or destroyed may obtain a duplicate by filing a form containing identifying information, notarized affidavit, and paying a fee.

2. (b) A registrant who wants his or her original certificate of registration or current certificate of reregistration issued in a name other than that under which the original was issued may secure a substitute certificate by submitting the following:

   1. (1) a duly executed affidavit;
   2. (2) the required fee;
   3. (3) a photograph properly identified;
   4. (4) the original of the certificate of registration.
