(a) Complaints alleging violations of the Act or Board rules should be in writing and accompanied by sufficient information or factual evidence to establish probable cause. Complaints shall be submitted on forms provided by the Board or in a written form that will provide the same information as the official form. Complaints must be notarized. Information to be provided includes:
