A replacement certificate will be issued to a registrant to replace one lost or destroyed provided:

1. (1) the current annual registration renewal is effective;
2. (2) the registrant makes proper request for such replacement certificate and submits an acceptable explanation of the loss or destruction of the original certificate; and
3. (3) the registrant pays the fee prescribed in §3.87 of this title (relating to Fees).
