The request shall include information on facilities and equipment as follows:

1. (1) Describe existing facilities that are available for the proposed program. Describe the present utilization of these facilities.
2. (2) What new facilities will be needed in the near future? Specify what special facilities and equipment will be needed and estimate their cost. From what sources do you anticipate obtaining needed facilities and equipment?
3. (3) Will the approval of this program result in planning for the addition of new facilities?
