1. (a) A licensee or permit holder shall notify the department of changes to any of following information:

   1. (1) change in the licensee's or permit holder's name no later than the effective date of the change;
   2. (2) change of the licensee's or permit holder's mailing or physical address no later than the effective date of the change; or
   3. (3) change in the licensee's drug testing policy no later than 30 days before the effective date of the change.
2. (b) The requirements of subsection (a)(3) apply only to a towing company regulated by these rules.
